Membership Participant Notice

This application is to obtain a valid membership to participate as a member of the Utah Soccer Association within the boundaries of the State of Utah for the current active season at the time of your registration. Registration applications are automatically accepted unless your previous record of play suggests repeated failure to abide by the rules of the game and the Association and its Affiliates or Leagues, in which case additional information may be needed or registration may be denied.

THE TEAM FOR WHICH YOU ARE REGISTERED IS AFFILIATED WITH THE UTAH SOCCER ASSOCIATION. YOU MUST BE REGISTERED VIA OUR ON-LINE REGISTRATION SYSTEM AND IN ORDER TO STAY ELIGIBLE FOR PLAY, YOU ARE REQUIRED TO PAY YOUR TOTAL REGISTRATION FEES IN A TIMELY MANNER, AS AGREED AT THE TIME OF THE REGISTRATION PROCESS. YOUR TEAM MUST ACCEPT YOUR REGISTRATION PRIOR TO YOU BECOMING ELIGIBLE TO PLAY. YOU WILL BE ELIGIBLE TO PLAY WHEN YOUR INFORMATION IS TRANSFERRED TO EACH GAME ROSTER AND YOU HAVE NOT BEEN CROSSED OFF. PLAYING ILLEGALLY WILL RESULT IN SANCTIONS AGAINST THE TEAM AND/OR PLAYER.

Please read all the instructions and contents of this form and the other forms included in your waiver package carefully, since accurate completion is necessary for proper validation of your liability insurance. The applicant may request a copy of the Participant Accident Plan benefits, which includes limitations and exclusions. This is available at the Utah Soccer Association office.

I acknowledge that I assume the risk for any personal injury, illness I sustain before, during or after a game and/or practice, and I will not hold liable any game official, player, team, club, league, State Association, the USSF, FIFA, or any other person or entity.

I hereby acknowledge that I have read the information above and accept the bylaws, rules, policies, procedures, etc. as described within this waiver. I KNOWINGLY ASSUME THE RISKS AND AM SIGNING THIS ACKNOWLEDGEMENT VOLUNTARILY.
